Telehealth consent

All care at this practice is provided by secure video or phone. Please read how telehealth works, what to expect, and what it asks of you.

What telehealth is

Telehealth means receiving psychiatric evaluation and medication management through live video or audio instead of in person. Lisa Jimenez, PMHNP-C, will use the same clinical judgment and standards of care as in an in-person visit.

Where you need to be

Lisa is licensed in Oregon and Washington. You must be physically located in Oregon or Washington during every visit. You will be asked to confirm your location at the start of each visit, and to share a phone number where you can be reached if the connection drops.

Possible benefits

  • Care from home, without travel or time away from work.
  • Easier scheduling and follow-up.
  • Access to a psychiatric prescriber when local options are limited.

Possible risks and limits

  • Technology can fail. The video or audio may drop, and a visit may need to continue by phone or be rescheduled.
  • Even with strong safeguards, electronic communication carries some risk of being intercepted or accessed by others.
  • A video visit cannot include a physical exam or lab tests. Lisa may recommend an in-person visit or another service if she thinks it is needed.
  • Telehealth is not right for every situation. Lisa may advise a different level of care at any time.

Your part

  • Join from a private place where you will not be overheard, using a secure internet connection and your own device.
  • Do not record or share visits without written permission from everyone involved. Lisa will not record visits without your written consent.
  • Be ready to confirm your identity when asked.
  • Do not drive or do anything that needs your full attention during a visit.

Privacy and security

Visits take place on Google Meet, using a practice account that is covered by a HIPAA Business Associate Agreement with Google and set up to protect health information. Your records are protected as described in the Notice of Privacy Practices.

Emergencies

This practice is not an emergency service. If you are in crisis or think you may hurt yourself or someone else, call or text 988, or call 911, or go to your nearest emergency room. Please do not wait for a reply from this practice. At your first visit, you will be asked for the address of where you are and an emergency contact, so help can be sent if it is ever needed.

Prescriptions

Prescriptions are sent electronically to the pharmacy you choose. Some medications, including controlled substances, are subject to additional federal and state rules, and may require extra steps or an in-person visit.

Fees

Telehealth visits are billed the same way as in-person visits. See Insurance & fees for what we accept and self-pay rates.

Cancellations and missed appointments

Please give at least 24 hours' notice to cancel or reschedule. A cancellation with less than 24 hours' notice, or a missed visit, may be charged $75 for an initial evaluation or $50 for a follow-up. Your first missed appointment is waived. Fees are waived for emergencies or circumstances beyond your control. If you miss several visits in a year, we will talk about an attendance plan, and discharge with referrals is a last resort. If you have not joined within 15 minutes and have not contacted the practice, the visit may be counted as missed. If you are having trouble connecting, call or text right away. This fee is not covered by insurance and does not apply to patients with Medicare or Apple Health. You will be asked to sign the cancellation policy form before your first visit.

Your choice

Telehealth is voluntary. You may decline it or stop at any time without affecting your right to future care. By scheduling and joining a visit, you confirm that you have read this information, that your questions have been answered, and that you agree to receive care by telehealth. You will be asked to confirm this consent before your first visit.
